Collection kits ship after your booking is verified. A blood draw of about 3 mL in an EDTA tube is standard; saliva kits are available for some assays. Keep the tube at the temperature listed on the kit insert and return it with the prepaid courier. If collection looks difficult, contact us first so we can plan the right tube and volume.

Coverage varies by plan and indication. On request we provide the test name, CPT codes and a medical-necessity letter so your insurer or employer can complete pre-authorization before collection. Final coverage decisions rest with your payer.

CPT codes depend on the assay and any add-on analysis for hereditary breast and ovarian cancer testing; the exact code set appears on your quote and lab requisition. Ask your counselor for the codes before submission so claims are not rejected.
